As a patient of Smiles for Life Orthodontics, you are automatically enrolled into a patient incentive program to earn rewards. Successful orthodontic treatment depends on patient cooperation and we will reward you for doing your part!

This is how you earn tokens:

Because keeping your teeth clean and healthy is such an important part of Orthodontic Care, we recommend you visit your dentist every 6 months for a check- up and cleaning.

Please print this DENTAL REWARD VOUCHER [PDF] [DOC] and take it to your next dentist appointment. Have your dentist complete the information and we will reward you with tokens.

Exam and cleaning (voucher) = 5 tokens
No cavities (voucher) = 5 bonus tokens
Good brushing = 1 token
No loose brackets = 1 token
Celebrate your birthday = 2 tokens
Wear your “Smiles” t-shirt to school and then to your appointment = 1 token
Get caught in your “Smiles” t-shirt by a staff member outside of the office = 5 tokens
(Shopping, movies, restaurant, etc… Staff member will give you a ticket to turn in at your next scheduled appointment to receive your tokens)
Refer a friend = 25 tokens

You can save your tokens or redeem them at any time. All you need to do is bring the tokens you would like to redeem to the front desk and ask one of our staff to help you.

Rules:

• Program only applies to patients
• Tokens are awarded for regularly scheduled appointments only (not emergency appointments).
• Gift cards and/or tokens are not redeemable for cash.
• Your account must be current in order to redeem tokens for a prize.
• You must bring the “Dental Reward Voucher” printed and signed by your dentist to your next scheduled orthodontist appointment in order to receive tokens.
